Remember to create a Google Form you must navigate (sign-in) to your Google Doc account and click on the "Create New" option and from the menu select "Form". Complete the form by selecting the question type, etc. and filling in the information. You can view the results of the quiz either as a spreadsheet or as graphs (click on Form>Show Summary of results). Remember you must set the sharing settings (top right) to public and allow edits before embedding your quiz. When using Google Docs it is best if you use either the FireFox or Google Chrome Browsers.
How to Embed your form/quiz on your wiki page
When your form/quiz is completed
1.Click on the "Form" tab at the top of the Google Form page
2.Click on "Embed"
3.Copy the code
4.Go to your wiki page
5.Click on the edit option
6.Click on the widget option in the Editor toolbar
7.Click on "other HTM"L
8.Paste the code in the text box
9.Click save.
10. The "Other Widget" symbol will appear on the page
11. Click "Save" in the Editor toolbar and the form/quiz will appear on your page!
